PS Audio have been around for 40 yrs, formed in Calif. originally by Paul McGowan & Stan Warren. If memory serves me correctly, their PS IV preamp was assembled in NZ during the 70s. was the iv nz assembled ?
i had one and it sounded pretty good, better in active mode rather than passive.
build quality was low.

I too had one of those, had completely forgotten about the active and passive modes....
Thought it was a pretty good sounding pre amp at the time. Had a separate power supply IIRC ?

tbh don't remember.
the over-riding memory was that the phono stage had such poor noise rejection that my sample could pick up a local radio station.

Can't remember what cart I was using at the time but don't recall that I had any issues in that regard. I think there were matching power amps somewhere too but I've never seen them.
